Ohio Rural Health Transformation Program
Ohio will receive more than $200 million from CMS through the Rural Health Transformation Program to strengthen health care in rural communities. Proposals are accepted from health care providers and organizations seeking to expand access and improve care quality in Ohio rural areas.

Eligibility
Ohio health care providers, hospitals, and organizations serving rural communities in Ohio. Must demonstrate capacity to improve access and care quality in rural areas.
Requirements
- Must be a health care provider or organization serving Ohio rural communities
- Must submit both a technical proposal and cost proposal
- Proposals accepted through state-announced application windows
