About this program
PACE provides comprehensive medical and social services to eligible adults age 55 and older through community-based centers and in-home support, helping participants delay or avoid nursing home care. Deadline notes: Enrollment is generally available through local PACE organizations subject to eligibility and service area. Notes: New Jersey expanded PACE availability statewide through county service areas.
Eligibility
Individuals age 55 or older who live in a PACE service area, are certified as needing nursing home level of care, and can live safely in the community with PACE support.
Requirements
- Contact a local PACE organization for screening; provide age, residency, Medicare or Medicaid information, clinical need, and financial documentation as required.
