Primary care physicians working in independent private practices in Tier 1 or Tier 2 counties of North Carolina's County Distress Rankings.
Requirements
- Must be a primary care physician licensed in North Carolina
- Practice must be an independent private practice
- Practice must be located in Tier 1 or Tier 2 distress-ranked NC county
