About this program
The Senior Wellness Pilot Program provides grant funding for local government and nonprofit entities to reduce social isolation and strengthen social connection among older adults and individuals with disabilities in New Jersey communities. Eligibility
Eligible New Jersey local government and nonprofit entities proposing initiatives for older adults and individuals with disabilities at risk of social isolation.
Requirements
- Submit the state grant application with project plan, budget, population served, social isolation strategy, staffing, outcomes, and required organizational documents.
